Why this applies to Naproxen, Sumatriptan

Naproxen, Sumatriptan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Treximet combines two medicines: naproxen (an anti-inflammatory pain reliever) and sumatriptan (a triptan for migraines). The concern here involves a liver enzyme called CYP1A2. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow this enzyme down, but only in lab dish (in vitro) studies. Nothing like this has actually been reported in people yet.

In theory, if that enzyme is blocked, sumatriptan could clear a bit more slowly and build up, which might mean more of its usual effects like chest or throat tightness, tingling, flushing, dizziness, or drowsiness. Because this is only theoretical lab data, I would not panic, but it is worth a quick chat with your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Treximet combines two medicines: naproxen (an anti-inflammatory pain reliever) and sumatriptan (a migraine medicine). The naproxen part is partly cleared by a liver enzyme called CYP2C9. In lab studies only, plumbagin from Ceylon Leadwort slowed this enzyme down. If that held true in people, naproxen could build up a bit, which might mean more of its usual side effects like stomach upset, heartburn, a higher risk of stomach irritation or bleeding, or fluid retention.

I want to be honest about how thin this evidence is. It comes from test-tube work, not from real patients, so it's theoretical for now. Still, it's worth mentioning Ceylon Leadwort to your pharmacist or doctor before combining them, since this plant isn't well studied in humans.

ANTICOAGULANT/ANTIPLATELET DRUGS

Naproxen, Sumatriptan also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Treximet combines naproxen (an NSAID pain reliever) with sumatriptan (a migraine medicine). The piece that matters here is the naproxen. Like other NSAIDs, naproxen already makes platelets a little less sticky and can irritate the stomach lining, which raises bleeding risk on its own.

The concern is simple: in animal studies, Ceylon Leadwort root extract also reduced platelet stickiness and made bleeding take longer. Stack that on top of naproxen and you could, in theory, see more bruising, nosebleeds, or stomach bleeding. To be fair, this hasn't been shown in people yet, so it's a cautious flag rather than a proven problem. Still, I'd check with your pharmacist or doctor before combining them, especially if you take this often.
