Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for your HIV medicine, nelfinavir (Viracept). Your liver uses an enzyme called CYP3A4 to help clear this drug from your body. A compound in Ceylon Leadwort, called plumbagin, has been shown to slow that same enzyme down, at least in lab dish studies. If that happened in a person, nelfinavir could build up higher than expected, which might mean more side effects like diarrhea, nausea, gas, or stomach upset.

I want to be honest about how thin the evidence is here. This is only seen in test tubes, never reported in real people, so it's theoretical. Still, because nelfinavir is part of a careful HIV regimen you don't want to disturb, please check with your pharmacist or doctor before adding this supplement.
