Why this applies to Niacin, Lovastatin

Niacin, Lovastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

This combo (Advicor) pairs niacin with lovastatin, and it's the lovastatin part we need to watch here. Your body relies on an enzyme called CYP3A4 to break lovastatin down. In lab tests, plumbagin from Ceylon Leadwort slowed that same enzyme. If that held true in people, lovastatin could build up higher than intended, which can raise the odds of muscle aches, weakness, and (rarely) more serious muscle breakdown that can affect the kidneys.

Here's the honest part: this is only a test-tube finding, and it hasn't been seen in real patients yet, so it's theoretical for now. I still wouldn't combine them on your own. Please check with your pharmacist or doctor first, especially if you notice any new muscle pain.
