Why this applies to pioglitazone, metformin

pioglitazone, metformin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the honest picture with ActoPlus Met (that's pioglitazone plus metformin). A compound in Ceylon Leadwort called plumbagin has slowed down the CYP3A4 enzyme, but only in lab dish studies, not in real people. Of the two drugs in your pill, pioglitazone is the one partly handled by CYP3A4. If that enzyme were blocked, pioglitazone could build up a little, which in theory might mean more fluid retention, swelling, weight gain, or a stronger drop in blood sugar. Metformin barely uses the liver this way, so it's largely unaffected.

I want to stress this is theoretical and based on weak, test-tube evidence, so don't be alarmed. Still, before adding this supplement, run it by your pharmacist or doctor first.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

ActoPlus Met combines two diabetes medicines, pioglitazone and metformin, that work together to lower your blood sugar. Here's the tricky part with Ceylon Leadwort: in animal studies, its root extract has gone both ways, sometimes raising blood sugar and sometimes lowering it. That makes it hard to predict. It could push your numbers too low (causing shakiness, sweating, dizziness, or confusion) or blunt your medicine and let your sugar run high.

To be clear, this has only been seen in lab and animal research, never in people, so it stays theoretical for now. Still, if you do use this supplement, check your blood sugar more often and loop in your pharmacist or doctor before changing anything.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
