Why this applies to Sunitinib

Sunitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with Ceylon Leadwort and your sunitinib. Your liver uses an enzyme called CYP3A4 to break sunitinib down and keep its levels in check. In lab studies, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. If that held true in people, sunitinib could clear out more slowly and build up higher than intended, which may mean stronger side effects like fatigue, mouth sores, diarrhea, high blood pressure, hand-and-foot skin reactions, or bleeding.

That said, this is only a theoretical concern based on test-tube work, not real cases in patients, so I don't want to alarm you. Still, sunitinib is serious cancer medicine, so please don't add this supplement without first checking with your oncologist or pharmacist.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Sunitinib Used For?

Brand name: Sutent

Sunitinib malate capsules and Sutent capsules are both approved to treat adult patients with gastrointestinal stromal tumor (GIST) after the cancer has progressed on or could not be tolerated with imatinib mesylate. Both are also used for advanced renal cell carcinoma (kidney cancer) and as adjuvant (preventive) treatment after surgery for adults at high risk of kidney cancer coming back. Additionally, both treat progressive, well-differentiated pancreatic neuroendocrine tumors (pNET) that cannot be surgically removed or have spread.
