Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (called endoxifen), and the enzyme CYP2D6 does most of that work. Ceylon Leadwort contains plumbagin, which in lab tests slowed CYP2D6 down. If that same thing happened in people, it could mean your body makes less of the active form, which might make tamoxifen work less well at protecting against or treating breast cancer.

Now, the honest part: this has only been seen in test tubes, never in actual patients, so it's theoretical right now. Still, tamoxifen is too important to gamble with. Please check with your oncologist or pharmacist before adding this supplement so they can guide you safely.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the tricky part with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (mostly endoxifen) using liver enzymes, and CYP2C9 plays a role in that processing. Ceylon Leadwort contains plumbagin, which in lab tests slowed down CYP2C9. The concern is that blocking this enzyme could change how much active drug you end up with, potentially weakening the cancer-fighting benefit you're counting on.

That said, I want to be honest about the evidence. This was only seen in test-tube and animal studies, never confirmed in real people, so it's theoretical at this point. Still, tamoxifen is too important to gamble with. Please check with your oncologist or pharmacist before adding this supplement so they can keep your treatment working as intended.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (mostly through an enzyme called CYP2D6, with some help from CYP3A4) before it can do its breast-cancer-fighting job. Ceylon Leadwort contains plumbagin, which in lab dishes has been shown to slow down the CYP3A4 enzyme.

In theory, slowing that enzyme could nudge how tamoxifen is processed, though the main activation step relies on a different enzyme, so the practical effect is uncertain. Importantly, this is only test-tube evidence, not something seen in real patients. Still, because tamoxifen is such a critical cancer medicine, I wouldn't gamble on it. Please check with your oncologist or pharmacist before adding this supplement.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Tamoxifen

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat metastatic breast cancer (breast cancer that has spread) in both women and men, and as adjuvant (after-surgery) therapy for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS) — a very early, non-invasive form of breast cancer — tamoxifen is used after breast surgery and radiation to reduce the risk of developing invasive breast cancer. It is also approved to reduce the incidence of breast cancer in women who are at high risk but have not yet been diagnosed. Soltamox (oral solution) is specifically indicated for all of these uses in adult patients.
