Tolterodine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist with tolterodine (Detrol): your liver uses an enzyme called CYP2D6 to help clear this medication. A compound in Ceylon Leadwort called plumbagin has been shown, in lab dish studies only, to slow that same enzyme down. If that happened in your body, tolterodine could build up a bit more than usual.

What might you notice? The classic antimuscarinic side effects could feel stronger, things like dry mouth, constipation, blurry vision, dizziness, or a faster heartbeat. I want to be honest, though: this is purely theoretical and has not been seen in real people yet, so I wouldn't lose sleep over it. Still, before adding this supplement, run it by your pharmacist or doctor so they can keep an eye on how you're feeling.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear tolterodine. A compound in Ceylon Leadwort, plumbagin, has been shown to slow down that enzyme, but so far only in lab dish (in vitro) studies, not in actual people. If it did the same thing in your body, tolterodine could build up a bit higher than usual.

What might that feel like? More of tolterodine's typical side effects, like dry mouth, constipation, blurry vision, or feeling drowsy or dizzy. Since this is only theoretical right now, I wouldn't panic. But before you pair this supplement with your bladder medicine, please run it by your pharmacist or doctor so they can keep an eye on you.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

Tolterodine — available as tablets (including Detrol and Tolterodine Tartrate tablets) and extended-release capsules (including Detrol LA and Tolterodine Tartrate extended-release capsules) — is prescribed for overactive bladder. It targets three main symptoms: a sudden, hard-to-control urge to urinate (urgency), urinating too often (frequency), and leaking urine when that urge hits (urge urinary incontinence).

All approved forms and brands share this same indication. No additional or separate uses are listed in the prescribing information for any individual product.
