Why this applies to Ziprasidone

Ziprasidone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the basic idea with these two together. Ziprasidone (Geodon) is partly cleared by a liver enzyme called CYP3A4. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to slow that enzyme down. If that same effect happened in people, your body might clear ziprasidone a little slower, so more could build up in your system.

What that could mean in practice is more of ziprasidone's usual effects, like drowsiness, dizziness, or feeling lightheaded when you stand. Ziprasidone can also affect heart rhythm (QT interval), so I'd be cautious. That said, this is only a theoretical concern from test-tube research, never reported in real patients, so please don't panic. Just loop in your pharmacist or doctor before combining them.

What Is Ziprasidone Used For?

Ziprasidone is prescribed for adults with schizophrenia and for adults experiencing manic or mixed episodes of bipolar I disorder. It can be used on its own for acute manic or mixed episodes, or added to lithium or valproate to help maintain stability in bipolar I disorder long-term.

An injectable form is also available for rapid control of agitation in people with schizophrenia who need quick relief. Prescribers are advised to weigh ziprasidone's potential effect on heart rhythm when choosing among treatment options.
