Why this applies to Imatinib

Imatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about Gleevec (imatinib): your body relies on a liver and gut enzyme called CYP3A4 to handle it. Imatinib is active on its own (it is not a prodrug), so anything that speeds up that enzyme tends to clear the drug faster and may leave less of it working against your cancer. In one small study in healthy men, taking Danshen for 10 to 14 days revved up CYP3A4 and lowered levels of a test drug quite a bit. That same effect could, in theory, weaken imatinib over time.

Oddly, a single dose of Danshen did the opposite and raised levels. Because imatinib is a serious cancer medicine, I'd play it safe and run any Danshen use past your oncologist or pharmacist first.

What Is Imatinib Used For?

Brand name: Gleevec

Imatinib treats a range of blood cancers and solid tumors. It is used for Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in adults and children — whether newly diagnosed or in more advanced phases — and for Philadelphia chromosome positive acute lymphoblastic leukemia (Ph+ ALL) in both adults and pediatric patients.

It is also used for Kit-positive gastrointestinal stromal tumors (GIST), including after surgical removal, and for several rarer conditions: myelodysplastic/myeloproliferative diseases (MDS/MPD) linked to specific gene changes, aggressive systemic mastocytosis (ASM), hypereosinophilic syndrome (HES) and/or chronic eosinophilic leukemia (CEL), and unresectable, recurrent, or metastatic dermatofibrosarcoma protuberans (DFSP).
