Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the quick version: your body uses a kind of cellular bouncer called P-glycoprotein to push sirolimus (Rapamune) out and help clear it. Danshen seems to slow that bouncer down. In a small study, danshen raised levels of another P-glycoprotein drug by roughly a third. If the same thing happens with sirolimus, more of it could build up in your blood.

That matters because sirolimus is a narrow-window drug, meaning small changes can tip you into too-high levels. Higher amounts can mean mouth sores, swelling, high cholesterol or blood sugar, low blood counts, or kidney strain. The evidence here is limited, not ironclad, but with a transplant drug I'd play it safe. Please don't start or stop danshen on your own. Talk with your transplant team or pharmacist first.

Sirolimus also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with sirolimus (Rapamune): your body clears it using an enzyme called CYP3A4, and Danshen can mess with how fast that enzyme works. In one small study, taking Danshen for about 10 to 14 days actually sped up CYP3A4. For sirolimus, that could mean your levels drop and the drug gets cleared too quickly, which is a real concern when the whole job of this medicine is to keep your immune system from rejecting your transplanted kidney.

Sirolimus is a narrow therapeutic index drug, so even modest shifts matter. Interestingly, a single dose of Danshen did the opposite and raised levels, so timing makes this unpredictable. Please don't start or stop anything on your own. Loop in your transplant team or pharmacist first, since they monitor your sirolimus blood levels closely.

What Is Sirolimus Used For?

Brand name: Rapamune

Sirolimus oral solution and tablets are used to help prevent organ rejection in kidney (renal) transplant patients aged 13 and older. They are also used to treat lymphangioleiomyomatosis (LAM) — a rare lung disease in which abnormal muscle-like cells invade lung tissue.

Fyarro (sirolimus, intravenous injection) is used to treat adults with locally advanced, unresectable, or metastatic malignant PEComa — a rare tumor that grows in soft tissue or organs. Hyftor (sirolimus topical gel) is used to treat facial angiofibroma (small benign skin growths on the face) associated with tuberous sclerosis in adults and children aged 6 and older.
