Ethionamide falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the simple version: both of these are tough on your liver, and stacking them could be like asking your liver to do double duty. Ethionamide is a second-line tuberculosis medicine that's already well known for raising liver enzymes and sometimes causing real liver injury, including symptoms like nausea, belly pain, yellowing of the skin or eyes, and unusual tiredness. Epistane is a methylated anabolic steroid that is also hard on the liver. Put them together and the strain may add up.

I'll be honest, this concern is based on how these substances behave, not on solid studies, so it's theoretical. Still, I'd steer well clear of Epistane in general (it isn't a legal or safe supplement), and definitely loop in your doctor or pharmacist if you're on Ethionamide.
