Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the wrinkle with codeine: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what really relieves pain and calms a cough. Fever bark contains a yohimbine-like compound that may block that same enzyme, at least in lab tests.

If that blocking happens in real life, less codeine would turn into morphine, so you might get weaker pain relief or less cough control than expected. The good news is this is only a theoretical, test-tube finding, and the effect of fever bark itself isn't well understood, so I wouldn't panic. Still, fever bark is a potent, poorly studied herb. Please run it by your pharmacist or doctor before combining the two.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.

Codeine Sulfate tablets are used to manage mild to moderate pain in adults when non-opioid pain treatments have not worked or are not tolerated. It is only appropriate when an opioid is truly needed.

Because of the serious risks of addiction, misuse, overdose, and death, codeine is reserved for patients whose pain cannot be managed adequately with other options. It is not a first-choice pain reliever.
