Macitentan falls under ANTIHYPERTENSIVE DRUGS — the group Melatonin interacts with.
Some clinical research suggests that taking melatonin decreases blood pressure in healthy adults. Also, melatonin seems to lower systolic and diastolic blood pressure in individuals with high blood pressure at nighttime or untreated essential hypertension. However, melatonin seems to worsen blood pressure in patients who are taking antihypertensive medications. Immediate-release melatonin 5 mg at night in combination with nifedipine GITS (Procardia XL) increases systolic blood pressure an average of 6.5 mmHg, diastolic blood pressure by an average of 4.9 mmHg, and heart rate by 3.9 bpm. Also, results from animal research suggest that melatonin reduces the effectiveness of certain antihypertensive drugs, including methoxamine and clonidine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with melatonin and Opsumit (macitentan). Macitentan helps relax the blood vessels in your lungs to ease pulmonary arterial hypertension, and it also has a mild blood-pressure-lowering effect throughout the body. The concern here isn't about how your liver breaks the drug down. Instead, it's about overlapping effects on blood pressure.
Interestingly, in some studies melatonin actually worked against certain blood pressure medicines and nudged blood pressure (and heart rate) up a bit. So with macitentan you could see blood pressure that's harder to predict, possibly more dizziness, lightheadedness, or fluid swelling. This is largely theoretical and effects are usually small, but since PAH is serious, please check with your pharmacist or doctor before combining them. Don't change anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Macitentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version on combining melatonin with macitentan (Opsumit). Your liver uses an enzyme called CYP3A4 to help clear macitentan from your body. Some lab research suggests certain batches of melatonin might slow that enzyme down a little. In theory, that could let macitentan build up slightly, which might nudge up its usual effects like lower blood pressure, headache, swelling in the legs, or a stuffy nose.
I want to be honest about the strength of this, though. It's rated minor and comes only from test-tube and lab studies, not from real people. For most folks it's unlikely to cause trouble. Still, since macitentan is a serious medication for your lungs, it's worth a quick chat with your pharmacist or doctor before adding melatonin.

What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.
In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
