Isoniazid, Rifampin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Milk Thistle interacts with.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for your combination tablet (isoniazid plus rifampin) used to treat tuberculosis. Milk thistle may block a transporter called OATP-B1, which helps pull certain drugs from your gut into your body. In theory, blocking it could lower how much of these TB medicines get absorbed, which might make them slightly less effective at controlling the infection. With TB, getting the full benefit of your medicines matters, so any drop in absorption is worth taking seriously.

That said, this concern rests only on lab and animal data, not real human studies with these exact drugs, so it's far from proven. I wouldn't lose sleep over it, but please don't add milk thistle on your own. Run it by your pharmacist or doctor first, especially since these TB drugs already affect the liver.
