Why this applies to Dolutegravir, Rilpivirine

Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the honest picture with Juluca (dolutegravir and rilpivirine) and moringa. The rilpivirine part of this combo is partly cleared by a liver enzyme called CYP3A4. Lab studies on moringa pull in opposite directions, with some hinting it blocks that enzyme (which could let drug levels rise) and others suggesting it speeds the enzyme up (which could lower levels and weaken your HIV control). We honestly don't know which way it would land in real life.

The good news is that this is theoretical, based on test-tube work, and a human study with a similar HIV drug showed no real change. Food amounts of moringa are unlikely to matter, but with HIV therapy you don't want surprises. Please run any moringa supplement past your pharmacist or doctor first.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the short version on combining moringa with Juluca (dolutegravir and rilpivirine). Your body uses a transporter called P-glycoprotein to help move these HIV medicines around and clear them. Lab studies suggest moringa leaf extract may slow that transporter down. In theory, that could let a little more of the drug build up, which might mean a bit more of the usual side effects like headache, nausea, trouble sleeping, or unusual dreams.

I want to be honest with you, though: this is based only on test-tube research, and it has not actually been seen in people. Neither of these drugs is a prodrug, so the direction would be toward slightly higher levels, not lost effect. Food-amounts of moringa are unlikely to matter, but concentrated supplements are less predictable. Since staying steady on HIV therapy is so important, please run any moringa supplement by your pharmacist or doctor first.
