Why this applies to Fostemsavir Tromethamine

Fostemsavir Tromethamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the situation with Rukobia (fostemsavir) and moringa. Your body partly relies on an enzyme called CYP3A4 to break down fostemsavir's active form. In the lab, moringa has pulled in two directions: some studies suggest it slows CYP3A4 down, while others suggest it speeds it up. So in theory it could nudge your fostemsavir levels up (more side effects like headache, nausea, or rash) or down (less HIV control). We honestly can't say which.

The good news is this is theoretical, based only on test-tube work, and a small human study with a similar HIV drug showed no real change. Food amounts of moringa are unlikely to matter, but concentrated supplements are less certain. Since keeping your HIV treatment working is so important, please check with your pharmacist or doctor before adding moringa.

Rukobia (fostemsavir) is used to treat HIV-1 infection in adults who have tried many other antiretroviral treatments and whose virus is no longer responding — due to resistance, side effects, or safety concerns. It is always used alongside other HIV medicines, not on its own.

Rukobia is specifically intended for heavily treatment-experienced patients with multidrug-resistant HIV-1 who are failing their current regimen. It is not a first-line HIV treatment.
