Why this applies to Methadone

Methadone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Your body relies on a liver enzyme called CYP3A4 to help clear methadone. Moringa may bump into that same enzyme, but here's the honest picture: lab studies disagree. Some suggest moringa slows CYP3A4 down (which could let methadone build up, raising the risk of heavy sedation, slowed breathing, or constipation), while others suggest it speeds the enzyme up (which could lower methadone levels and nudge you toward withdrawal feelings).

Methadone is not a prodrug, so it works directly. The good news is this concern comes only from test-tube and animal data, and a human study with a related drug saw no real change. Food amounts of moringa are unlikely to matter, but since methadone has a narrow safety window, please run any concentrated moringa supplement past your pharmacist or doctor first.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
