Why this applies to Nelfinavir Mesylate

Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the situation with moringa and your HIV medication, nelfinavir (Viracept). Nelfinavir is handled by a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is that the lab studies don't agree: some hint that moringa slows the enzyme down (which could let nelfinavir build up and possibly cause more stomach upset or diarrhea), while others suggest it speeds the enzyme up (which could lower nelfinavir levels and make it work less well against the virus).

This is all theoretical, based on test-tube research, not real patients. One small human study with a similar drug showed no change. Still, because keeping HIV under control matters so much, please run it by your pharmacist or doctor before adding moringa supplements.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with moringa and your HIV medication Viracept (nelfinavir). Nelfinavir is moved around the body by a transporter protein called P-glycoprotein. In lab dish studies, moringa leaf extract seemed to block that transporter. If that same thing happened in people, it could let a bit more nelfinavir build up, which might mean stronger effects and more of its usual side effects, like diarrhea, nausea, gas, or stomach upset.

The important part: this is only a theoretical concern from test-tube research. It has not actually been reported in real patients, and the food amounts of moringa you'd cook with are very unlikely to cause trouble. Concentrated supplement doses are the bigger unknown. Since nelfinavir is a key part of keeping HIV controlled, please check with your pharmacist or doctor before adding a moringa supplement.
