Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with moringa and your pioglitazone (Actos). Your body uses a liver enzyme called CYP3A4 to help break down pioglitazone, and moringa might nudge that enzyme one way or the other. The catch is that lab studies disagree. Some test-tube work suggests moringa slows CYP3A4 down (which could let pioglitazone build up a bit, possibly more fluid retention, swelling, or low blood sugar when combined with other diabetes meds), while other research suggests it speeds the enzyme up (which could weaken the drug a little).
Honestly, this is theoretical and based on lab data, not real patients, so I wouldn't panic. Food-amounts of moringa likely matter even less than concentrated capsules. Still, since you're treating diabetes, mention any moringa supplement to your pharmacist or doctor and keep an eye on your blood sugar.

Animal research shows that moringa can lower blood glucose levels. However, research in humans has not shown consistent blood glucose lowering effects.

Here's the simple version: moringa may have a mild blood sugar lowering effect of its own. Since pioglitazone (Actos) also works to bring your glucose down, in theory the two together could nudge your levels lower than either alone. If that happened, you might notice signs of low blood sugar such as shakiness, sweating, dizziness, or feeling unusually hungry or tired.
I want to be honest about how weak this evidence is, though. The blood sugar drop from moringa has mostly shown up in animal studies, and human results have been all over the map. Food amounts, like moringa in a meal, are very unlikely to cause trouble. If you're taking concentrated supplements, it's worth a quick chat with your pharmacist or doctor and keeping an eye on your home glucose readings.

Pioglitazone tablets — sold as Actos and under the generic names Pioglitazone and Pioglitazone Hydrochloride — are used to improve blood sugar (glucose) control in adults with type 2 diabetes. They are taken along with diet and exercise, either alone or combined with other diabetes medicines such as a sulfonylurea, metformin, or insulin.
Pioglitazone is not for type 1 diabetes or diabetic ketoacidosis (a serious complication where the blood becomes too acidic). It only works when the body is still producing some of its own insulin.
