Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

If you're taking Vosevi to treat hepatitis C, here's the honest picture on moringa. Velpatasvir and voxilaprevir (two of the three medicines in Vosevi) are partly handled by a liver enzyme called CYP3A4. In the test tube, moringa has pulled in both directions: some studies suggest it slows this enzyme, others suggest it speeds it up. So we genuinely can't predict which way it would go in a real person.

This matters because finishing your full hepatitis C course at full strength is what clears the virus. If moringa lowered your drug levels, it could in theory make treatment less effective. The good news is this is theoretical, based on lab work, not patient outcomes. Still, while you're on Vosevi, I'd skip concentrated moringa supplements and check with your pharmacist or doctor first.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the situation with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir). All three of these antiviral ingredients are moved around your body by a transporter called P-glycoprotein, a kind of cellular pump. Lab studies suggest moringa leaf extract might slow that pump down. If that happened, more of the medication could build up in your blood, which in theory could mean stronger side effects like headache, tiredness, nausea, or diarrhea.

I want to be honest about how soft this evidence is. It comes only from test-tube research, and nothing like this has actually been reported in people taking Vosevi. Food amounts of moringa are very unlikely to matter, but concentrated supplements are less predictable. Since you're treating hepatitis C, it's worth a quick chat with your pharmacist or doctor before adding moringa.

Here's the good news on this one: the concern with moringa is mostly on paper, not from real patient cases. In lab dishes, moringa extract seems to speed up an enzyme called CYP1A2. The worry is that revving up this enzyme could clear certain medicines a bit faster and weaken them. With Vosevi, that would theoretically mean less of the medicine sticking around to fight the hepatitis C virus.

That said, Vosevi isn't a prodrug, and these three ingredients aren't mainly handled by CYP1A2, so any real-world effect is likely small. Still, finishing a full course of Vosevi at full strength matters for clearing the virus, so I'd loop in your pharmacist or doctor before adding concentrated moringa supplements during treatment.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
