Why this applies to Tepotinib

Tepotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the short version on combining moringa with tepotinib (Tepmetko). Your body clears tepotinib using a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is that the lab studies disagree: some hint moringa slows the enzyme (which could let tepotinib build up and raise the chance of side effects like nausea, swelling, diarrhea, or liver irritation), while others suggest it speeds the enzyme up (which could lower tepotinib levels and make it work less well against the cancer).

Honestly, this is theoretical and based only on test-tube data, with no real human study on tepotinib itself. A small human study with another drug showed no change at all. Food amounts in cooking are unlikely to matter, but concentrated supplements are a bigger unknown. Because tepotinib is a cancer medicine, please check with your oncologist or pharmacist before adding moringa.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist for your lung cancer pill, tepotinib (Tepmetko). Your body uses a little 'pump' called P-glycoprotein to help move this drug around and clear some of it. Lab studies (in test tubes, not in people) suggest moringa leaf extract may slow that pump down. If the pump works less, more tepotinib could stay in your system, which in theory could mean stronger effects and a higher chance of side effects like swelling in your legs, nausea, diarrhea, tiredness, or liver irritation.

I want to be honest: this is theoretical. It has never actually been reported in humans, and a sprinkle of moringa in food is very different from a concentrated capsule. Still, because cancer drugs leave little room for surprises, please run any moringa supplement by your oncologist or pharmacist first.

Tepmetko (tepotinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — lung cancer that has spread — whose tumors carry a specific genetic alteration called a MET exon 14 skipping mutation. A test on tumor tissue or blood is required to confirm this mutation before treatment begins.

This is a targeted therapy, meaning it works against a specific molecular driver of cancer growth rather than attacking all rapidly dividing cells.
