Neem and Ramelteon

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Neem interacts with whole categories of medication. Ramelteon falls into 3 of those categories, so it can interact with Neem in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Ramelteon

Ramelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Rozerem (ramelteon): your liver relies on an enzyme called CYP1A2 to clear it from your system. Neem leaf extract, at least in test-tube studies, seems to slow that same enzyme down. If that played out in real life, ramelteon could linger longer and hit a bit harder. That might mean stronger next-morning grogginess, lingering drowsiness, or feeling foggy and dizzy.

The big caveat: this is theoretical only. It comes from lab work, and nobody has actually reported it happening in people taking neem. So I wouldn't lose sleep over it, but I also wouldn't ignore it. If you're using neem regularly and taking ramelteon, just give your pharmacist or doctor a quick heads-up so they can keep an eye on how you're doing.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Ramelteon also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Here's the simple version. Your body clears Ramelteon mainly through a liver enzyme called CYP3A4. In test-tube studies, neem leaf extract seems to slow that same enzyme down. If that happened in your body, Ramelteon could hang around longer and hit harder than expected, which might mean feeling extra drowsy, groggy in the morning, or a bit dizzy when you get up at night.

The honest part: this is only seen in lab studies, not in real people, so it's more of a theoretical heads-up than a proven problem. Food or dental amounts of neem are unlikely to matter much, but concentrated supplements are a different story. Before you pair neem with Rozerem, check with your pharmacist or doctor.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

3 of 3

Ramelteon also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version for Rozerem (ramelteon). Your body uses a liver enzyme called CYP2C9 to help break this sleep aid down. Lab studies suggest neem leaf extract may slow that enzyme. If it does, ramelteon could linger a bit longer and feel stronger, which might mean more next-morning grogginess, drowsiness, or dizziness when you get up.

I want to be honest about the strength of this concern: it comes only from test-tube research, and nothing like it has been reported in actual people. So this is more of a theoretical caution than a proven problem. Still, since neem can be hard on the body in concentrated doses, it's worth a quick chat with your pharmacist or doctor before combining the two.

“Ramelteon (Rozerem) is a prescription sleep aid, and its 228 interacting supplements call for real care, starting with the nine rated major. The frustrating part is that the things sleepless people typically reach for, kava, CBD, a glass of wine before bed, sit at the heart of the problem.”

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
