Oolong Tea and Theophylline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

THEOPHYLLINE

Theoretically, oolong tea might increase the levels and adverse effects of theophylline.
Oolong tea contains caffeine. Caffeine decreases theophylline clearance 23% to 29%.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know if you drink oolong tea while taking theophylline: they're chemical cousins. Theophylline is actually very close in structure to caffeine, and your body clears them in similar ways. The caffeine in oolong tea can slow how fast you clear theophylline (studies suggest roughly a 23 to 29 percent drop in clearance), so the drug can quietly build up a little higher than expected.

Why does that matter? Theophylline has a narrow safety window, meaning small changes can tip you into side effects like jitteriness, a racing or pounding heart, nausea, trouble sleeping, or feeling on edge. A cup or two now and then is unlikely to cause trouble for most people, but regular or large amounts could add up. This concern is mostly theoretical, based on how caffeine behaves. Please check with your pharmacist or doctor before changing anything.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

This page covers Oolong Tea only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Theophylline page lists 126 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), Ephedra (major), Sida Cordifolia (major), Adrafinil (moderate) and African Wild Potato (moderate).

By severity: 3 major · 89 moderate · 34 minor.

From our pharmacist’s overview on the Theophylline page

“Stop ephedra before anything else if you take theophylline (Theo-Dur, Slo-Bid, and others) for asthma or COPD. Both are stimulants, and together they raise the risk of serious heart and nervous system effects; Sida cordifolia, an herb that contains ephedrine, carries the same major rating.”

About Oolong Tea

What is Oolong Tea?

  • Weight management
  • Mental alertness and energy
  • Heart health
  • Blood sugar support
  • Antioxidant support

Oolong tea is a traditional partially oxidized tea made from the same plant as green and black tea, and it provides caffeine and plant antioxidants. As a beverage it is generally considered safe for most healthy adults in moderate amounts, but evidence for specific health benefits like weight loss or blood sugar control is limited. If you have heart, blood pressure, or caffeine-sensitivity concerns, talk with your pharmacist or doctor.
