Why this applies to Abiraterone

Abiraterone falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing these two: abiraterone (Zytiga) can already be tough on the liver, which is why your doctor checks your liver enzymes regularly while you're on it. Ostarine has its own track record of liver trouble. The FDA has flagged SARMs like this for liver injury, and some research shows ostarine can push up ALT, one of the markers we watch to spot liver stress.

Stacking two things that each strain the liver could mean a higher chance of liver problems, with warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. To be fair, this interaction is mostly theoretical, based on how each one behaves rather than studies of the two together. Even so, since you're fighting prostate cancer, please don't add ostarine without talking to your oncologist or pharmacist first.

Abiraterone also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist for this pairing. Abiraterone (Zytiga) is one of the medicines that can slow down a liver enzyme called CYP3A4. Ostarine is partly cleared by that same enzyme, so in theory abiraterone could cause a bit more ostarine to linger in your body, which could nudge up its effects and its risks.

That said, the real-world signal here is weak. When researchers paired ostarine with a much stronger CYP3A4 blocker (itraconazole), ostarine levels barely budged, so abiraterone is unlikely to matter much. The bigger concern is ostarine itself, which is an unapproved SARM tied to liver injury, something that already deserves caution while you're on prostate cancer treatment. Please loop in your oncologist or pharmacist before combining them.

It is not approved by the FDA, is banned in sports, and has been linked to liver injury and other serious harms, so it is not recommended for general use.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
