Aprocitentan falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with combining these two. Aprocitentan (Tryvio) is a newer blood pressure medicine that already carries some risk of affecting the liver, which is why doctors check liver enzymes with it. Ostarine, a SARM, has its own track record of raising liver markers like ALT and has even been tied to reports of drug-induced liver injury by the FDA.
Stacking two things that can both stress the liver may add up. This isn't a direct chemical clash, and the evidence here is mostly theoretical based on how each behaves, but it's a real-world caution. Watch for warning signs like yellowing of the skin or eyes, dark urine, unusual tiredness, or belly pain, and check in with your doctor or pharmacist before mixing these.

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Ostarine is partially metabolized by CYP3A4. Clinical research shows that taking rifampin, a potent inducer of CYP3A4, reduces the maximum plasma concentration of ostarine by 23% and the area under the curve by 43%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing to know about this pairing: the concern runs one direction. Aprocitentan (Tryvio) is a blood pressure medicine that can nudge up the activity of the CYP3A4 enzyme, and Ostarine happens to be partly cleared by that same enzyme. So in theory, taking them together could speed up how fast your body breaks down Ostarine, leaving less of it around. In a study with rifampin (a strong version of this same effect), Ostarine levels dropped noticeably.
To be clear, this doesn't harm your Aprocitentan or your blood pressure control. It just means the Ostarine may do less. Honestly, Ostarine is an unapproved SARM linked to liver injury, so I'd talk with your doctor or pharmacist before using it at all.

Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the short version: this pairing is more about the SARM than your blood pressure pill. Aprocitentan (Tryvio) can act as a mild blocker of the liver enzyme CYP3A4, which is one of the enzymes that partly breaks down Ostarine. In theory, slowing that breakdown could let a little more Ostarine build up, which might mean stronger effects and a higher chance of its known harms, like liver stress or unwanted hormonal changes.
In real life, though, this is unlikely to matter much. When researchers combined Ostarine with a far stronger CYP3A4 blocker, its levels barely moved. The bigger issue is that Ostarine itself is an unapproved SARM linked to liver injury, so I'd really encourage you to talk with your doctor or pharmacist before using it.

Lowering blood pressure reduces the risk of serious cardiovascular events such as strokes and heart attacks. Tryvio is not a first-choice blood pressure medicine; it is intended for patients with hard-to-control high blood pressure who need an additional option.
