Erdafitinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Ostarine interacts with.

Theoretically, concomitant use of ostarine with CYP3A4 inducers could decrease the clinical effects of ostarine.
Ostarine is partially metabolized by CYP3A4. Clinical research shows that taking rifampin, a potent inducer of CYP3A4, reduces the maximum plasma concentration of ostarine by 23% and the area under the curve by 43%.

Here's the thing to understand about this pairing: the interaction runs in one direction. Erdafitinib (Balversa) can act as a CYP3A4 inducer, meaning it can speed up that enzyme. Since Ostarine is partly broken down by CYP3A4, taking it alongside Erdafitinib could clear the Ostarine out of your system faster and lower its levels. In a study with a strong inducer (rifampin), Ostarine blood levels dropped noticeably. So if anything, the Ostarine may simply work less well. Erdafitinib itself isn't a prodrug, so its cancer-fighting effect isn't the worry here.

The bigger concern is Ostarine itself. It's an unapproved SARM linked to liver injury, and you're already on a serious cancer therapy. Please loop in your oncologist or pharmacist before mixing the two.

Here's the gist for this combination. Erdafitinib (Balversa) can mildly block an enzyme called CYP3A4, which is one of the pathways that helps your body break down Ostarine. In theory, slowing that pathway could let Ostarine build up a little, which might mean slightly stronger effects or more of its known harms, like liver strain.

In real life, this is unlikely to amount to much. When researchers paired Ostarine with itraconazole, a far stronger CYP3A4 blocker than erdafitinib, the Ostarine levels barely budged. So the bigger worry isn't this specific interaction. It's that Ostarine is an unapproved SARM tied to liver injury, and you'd be using it alongside cancer treatment. Please loop in your oncologist or pharmacist before combining anything with erdafitinib.

Erdafitinib also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, ostarine might increase levels of drugs metabolized by CYP2C9, although clinical research suggests this is not clinically relevant.
Although in vitro research suggests that ostarine inhibits CYP2C9, more robust clinical research shows that ostarine does not significantly affect CYP2C9.

Here's the gist for Erdafitinib (Balversa) and Ostarine. Your bladder cancer medication is partly cleared by a liver enzyme called CYP2C9. In test tube studies, Ostarine looked like it might slow that enzyme down, which could theoretically let erdafitinib build up and make side effects (like high phosphate levels, mouth sores, dry eyes, nail or skin changes, or diarrhea) more likely.

The reassuring part is that better human studies showed Ostarine does not meaningfully affect this enzyme, so a real-world problem is unlikely. That said, Ostarine is an unapproved SARM tied to liver injury, and your cancer drug is serious business with its own monitoring. Please loop in your oncology team or pharmacist before combining anything with Balversa.

About Erdafitinib

What Is Erdafitinib Used For?

Brand name: Balversa

Balversa (erdafitinib) is used to treat adults with locally advanced or metastatic urothelial carcinoma (UC) — cancer of the bladder or urinary tract — that has spread and has not responded to, or has gotten worse after, at least one prior round of systemic treatment. Patients must have a specific type of FGFR3 genetic alteration in their tumor, confirmed by an FDA-approved test before starting.

Balversa is not recommended for patients who are eligible for — but have not yet received — a PD-1 or PD-L1 inhibitor (a type of immunotherapy). Those therapies should be tried first when appropriate.
