Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ostarine interacts with.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist for this combo. Nelfinavir (Viracept) is one of those HIV medicines that puts the brakes on the CYP3A4 enzyme, which is partly how your body clears Ostarine. On paper, slowing that breakdown could let a little more Ostarine build up, which might raise the chance of Ostarine's own side effects, including liver strain. The good news is that the real-world signal here is weak. When researchers paired Ostarine with itraconazole, a very strong CYP3A4 blocker, the Ostarine levels barely budged.

So this is mostly theoretical and rated minor. The bigger concern, honestly, is that Ostarine is an unapproved SARM tied to liver injury, and you don't want anything competing with your HIV therapy. Please loop in your doctor or pharmacist before mixing the two.
