Pioglitazone falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing these two. Ostarine (a SARM, not a true supplement) has been linked to liver injury, and clinical reports show it can push up ALT, a blood marker of liver stress. Pioglitazone (Actos) can also affect the liver in rare cases, so stacking two things that strain the same organ may add up.

This isn't about Ostarine changing how pioglitazone is broken down. It's simply two liver-stressing agents together. Watch for warning signs like yellowing skin or eyes, dark urine, nausea, or pain in your upper right belly. Keep in mind this caution is theoretical, based on how each acts, not proven in studies. Ostarine is unapproved and risky on its own, so please talk with your pharmacist or doctor before combining them.

Ostarine (also called MK-2866 or enobosarm) is an experimental, unapproved drug known as a SARM (selective androgen receptor modulator), not a true dietary supplement. It is not approved by the FDA, is banned in sports, and has been linked to liver injury and other serious harms, so it is not recommended for general use.

What Is Pioglitazone Used For?

Brand name: Actos

Pioglitazone tablets — sold as Actos and under the generic names Pioglitazone and Pioglitazone Hydrochloride — are used to improve blood sugar (glucose) control in adults with type 2 diabetes. They are taken along with diet and exercise, either alone or combined with other diabetes medicines such as a sulfonylurea, metformin, or insulin.

Pioglitazone is not for type 1 diabetes or diabetic ketoacidosis (a serious complication where the blood becomes too acidic). It only works when the body is still producing some of its own insulin.
