Why this applies to Voriconazole

Voriconazole falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern when you pair these two: voriconazole already puts some strain on your liver, and your doctor may even check liver blood tests while you're on it. Ostarine, a SARM, has its own track record of liver trouble. Real cases have shown raised liver enzymes (like ALT) and even drug-induced liver injury linked to it. Stack a liver-stressing supplement on top of a liver-stressing antifungal, and you may simply be doubling up on that risk.

To be fair, this is a theoretical caution based on how each one behaves, not a proven combo reaction. Still, watch for yellowing skin or eyes, dark urine, belly pain on the right side, nausea, or unusual tiredness. SARMs aren't FDA-approved, so I'd really suggest talking with your doctor or pharmacist before combining them.

Voriconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the simple version: in this pairing, voriconazole is the one doing the blocking. It's a strong CYP3A4 inhibitor, and Ostarine is partly broken down by that same CYP3A4 pathway. So in theory, voriconazole could slow Ostarine's breakdown and let a little more of it build up, which might mean slightly stronger Ostarine effects and side effects.

That said, the real-world evidence points the other way on how much this matters. When researchers paired Ostarine with itraconazole (another powerful CYP3A4 blocker much like voriconazole), Ostarine levels barely budged. So this is mostly theoretical and rated as minor and unlikely. My bigger concern is Ostarine itself, since it's an unapproved SARM tied to liver injury, and voriconazole can also stress the liver. Please loop in your doctor or pharmacist before combining them.

Voriconazole also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, ostarine might increase levels of drugs metabolized by CYP2C9, although clinical research suggests this is not clinically relevant.
Although in vitro research suggests that ostarine inhibits CYP2C9, more robust clinical research shows that ostarine does not significantly affect CYP2C9.

Here's the short version: your antifungal, voriconazole, is partly cleared by a liver enzyme called CYP2C9. Early lab (test tube) work hinted that Ostarine might slow that enzyme down, which in theory could let voriconazole build up a bit and raise the odds of side effects like vision changes, nausea, or liver strain. The good news is that better human studies didn't show Ostarine meaningfully affecting CYP2C9, so this concern looks mostly theoretical.

That said, I want to be honest with you. The bigger issue isn't this interaction, it's Ostarine itself. It's an unapproved SARM that's been tied to liver injury, and voriconazole can already be tough on the liver. Stacking two liver stressors isn't smart. Please run this by your doctor or pharmacist before combining them.

It is not approved by the FDA, is banned in sports, and has been linked to liver injury and other serious harms, so it is not recommended for general use.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
