Abrocitinib falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's something worth knowing about this pair. Abrocitinib (Cibinqo) isn't an anticoagulant, but it can lower your platelet count, especially in the first month or two of treatment. That matters because Puravive contains several ingredients (Panax ginseng, propolis, kudzu, and phellodendron) that may mildly reduce how well platelets clump together to form clots.

So the concern is overlap. If your platelets are already running low from abrocitinib, adding something that gently nudges them in the same direction could, in theory, make easy bruising or minor bleeding a little more likely. To be fair, this is based mostly on lab and animal studies, not solid human data, so the real-world risk looks small. Still, please run it by your pharmacist or doctor before combining them, particularly since abrocitinib already calls for blood count monitoring.

Abrocitinib also falls under Cytochrome P450 2C9 (Cyp2C9) Substrates.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist: Puravive contains quercetin, which can gently slow down a liver enzyme called CYP2C9. Your eczema medication, abrocitinib (Cibinqo), is partly cleared by that same enzyme. So if you take them together, abrocitinib may clear a bit more slowly and build up somewhat in your system. That could mean a stronger effect than intended, and potentially more side effects like nausea, headache, acne, or a higher chance of infections since this drug already dials down your immune system.

To be fair, this is based on modest evidence (a small study using concentrated quercetin, not Puravive itself), so it's a possible concern, not a guaranteed one. Still, I'd run it past your pharmacist or doctor before combining them.

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is approved to treat adults and adolescents aged 12 and older who have moderate-to-severe atopic dermatitis (a chronic form of eczema) that has not been adequately controlled with other systemic treatments, including biologic medications, or when those treatments are not appropriate.

It is considered a refractory treatment option — meaning it is used when the disease has been difficult to control with other available therapies. Cibinqo is not recommended to be used alongside other JAK inhibitors, biologic immunomodulators, or other immunosuppressants.
