Aspirin, Hydrocodone falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the part that matters for your Alor (aspirin plus hydrocodone): the aspirin half already thins your blood by quieting your platelets, the cells that help you clot. Puravive packs in a few ingredients (Panax ginseng, propolis, phellodendron's berberine, kudzu) that may gently nudge platelets in the same direction. Stack them and you could, in theory, bruise more easily or bleed a little longer from a cut, nosebleed, or your gums.

I'll be honest, this is mostly lab-dish and animal evidence, not proven in people, so I wouldn't panic. The hydrocodone side isn't really part of this concern. Still, since aspirin and added bleeding risk don't always mix well, run Puravive past your pharmacist or doctor before adding it.

Cytochrome P450 2D6 (Cyp2D6) Substrates

Aspirin, Hydrocodone also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the wrinkle with this combo: the hydrocodone in your Lortab ASA is actually a prodrug. Your body uses the CYP2D6 enzyme to turn part of it into hydromorphone, which adds to its pain-relieving punch. Puravive contains Panax ginseng, which might slow CYP2D6 by a tiny amount (roughly 6% in one study). If anything, that would mean a touch less of the drug gets converted, so in theory you could feel slightly less pain relief, not more.

Honestly, an effect this small is unlikely to be noticeable, and the evidence here is weak and mixed. The aspirin part isn't really affected by this at all. I wouldn't lose sleep over it, but if you take this regularly for pain, it's worth a quick chat with your pharmacist or doctor.

Cytochrome P450 3A4 (Cyp3A4) Substrates

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with your aspirin and hydrocodone combo. The hydrocodone part is handled in your liver partly by an enzyme called CYP3A4, and Puravive contains Panax ginseng, which can nudge that enzyme either way. The catch is the research pulls in opposite directions, so we can't say for certain what happens. If ginseng slowed that enzyme, more hydrocodone could linger, meaning extra drowsiness, dizziness, constipation, or slowed breathing. If it sped the enzyme up, you might get a bit less pain relief.

The good news is this is rated a minor interaction on weak evidence, so big problems are unlikely. Still, an opioid is nothing to gamble with. Run it by your pharmacist or doctor before pairing them, especially if you feel unusually sleepy.
