Avatrombopag Maleate falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist: Puravive contains quercetin, and one of avatrombopag's ingredients (Doptelet) gets moved around your gut by a transporter called P-glycoprotein. There is some early, lab-style evidence that quercetin can slow that transporter down. If that happens, slightly more avatrombopag could get absorbed, which in theory could nudge your platelet count higher than expected. With this drug, that might mean a small risk of an overly strong response. Avatrombopag isn't a prodrug, so we're talking about more drug effect, not less.

Honestly, this is rated minor and rests on weak, theoretical evidence, so I wouldn't panic. Still, since avatrombopag is dosed carefully and platelet levels matter, run Puravive past your pharmacist or doctor first.

About Avatrombopag

What Is Avatrombopag Used For?

Brand name: Doptelet

Avatrombopag treats thrombocytopenia (a low platelet count that raises bleeding risk) in three groups of patients. Doptelet tablets are used in adults with chronic liver disease who need their platelet count raised before a medical procedure, and in adults with chronic immune thrombocytopenia (ITP) whose platelet count has not responded well enough to earlier treatments. Doptelet tablets are also used in children aged 6 years and older with persistent or chronic ITP who have not responded to prior treatment.

Doptelet Sprinkle (oral granules) is specifically indicated for children aged 1 year to less than 6 years with persistent or chronic ITP who have not responded adequately to a previous treatment.
