Why this applies to Digoxin

Digoxin falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the practical piece for you. Puravive contains quercetin, and digoxin happens to leave your body partly through a pump in your gut and kidneys called P-glycoprotein. In a test tube, quercetin can slow that pump down. If that happened in your body, a little more digoxin could stay in your bloodstream than expected.

Why that matters: digoxin has a narrow safety window, so even small upward shifts can edge you toward side effects like nausea, loss of appetite, blurry or yellow-tinged vision, or a slow or irregular heartbeat. That said, this is rated a minor, theoretical concern, and human studies on quercetin have been mixed (sometimes levels go up, sometimes nothing changes). I wouldn't panic, but because digoxin is so dose-sensitive, please run Puravive past your pharmacist or doctor first.

About Digoxin

What Is Digoxin Used For?

Brand names: Lanoxicaps, Lanoxin, Digitek

Digoxin tablets, oral solution, and injections (including Lanoxin brand) are all approved to treat mild to moderate heart failure in adults — improving symptoms like exercise tolerance and reducing hospitalizations, though it does not extend life. It is also approved to control the resting ventricular rate (how fast the lower chambers beat) in adults with chronic atrial fibrillation. The oral solution specifically notes it should not be used for multifocal atrial tachycardia.

In children with heart failure, digoxin is used to strengthen the heart's contractions (increase myocardial contractility). Digoxin is best used alongside a diuretic (water pill) and an ACE inhibitor when possible.
