Ethinyl Estradiol, Desogestrel falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist for your birth control pill (the ethinyl estradiol and desogestrel combo). Puravive contains quercetin, which can mildly slow down a liver enzyme called CYP2C9. Since hormones in your pill are partly processed by similar liver pathways, blocking that enzyme could let a bit more estrogen linger in your system than usual.

In plain terms, that might mean slightly more of the typical estrogen-related effects, like nausea, breast tenderness, headaches, or bloating. This is a theoretical concern, though, mostly based on a small study using a different drug, so it's far from certain. Concentrated supplement doses matter more than quercetin you'd get from food. Before pairing Puravive with your pill, please run it by your pharmacist or doctor first.

Cytochrome P450 3A4 (Cyp3A4) Substrates

Ethinyl Estradiol, Desogestrel also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with your birth control pill (the ethinyl estradiol and desogestrel combo). Puravive contains Panax ginseng, which can nudge a liver enzyme called CYP3A4, the same one your body uses to process the hormones in your pill. The tricky part is that studies disagree: in some cases ginseng seems to speed this enzyme up, and in others it slows it down. We honestly can't predict which way it would go for you.

If ginseng sped up that enzyme, your hormone levels could dip a bit, which in theory could mean breakthrough bleeding or slightly less reliable protection. The good news is this is rated a minor, weakly-supported interaction, so a serious problem is unlikely. Still, please run Puravive by your pharmacist or doctor first, since steady birth control levels matter.

P-Glycoprotein Substrates

Ethinyl Estradiol, Desogestrel also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.
