Why this applies to Glipizide, Metformin

Glipizide, Metformin falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

This combo pill pairs glipizide with metformin. The piece that matters here is the glipizide. Your liver clears it using an enzyme called CYP2C9, and Puravive contains quercetin, which can mildly slow that enzyme down. If glipizide leaves your body a little slower, more of it can hang around and push your blood sugar lower than expected.

In real life, that could mean low blood sugar (hypoglycemia): shakiness, sweating, dizziness, hunger, or feeling foggy. The metformin part isn't affected by this enzyme. The evidence is fairly limited (drawn from a small quercetin study), so I wouldn't panic, but I would mention Puravive to your pharmacist or doctor and keep an eye on your sugar readings if you start it.

Puravive contains Panax ginseng. Clinical studies indicate that Panax ginseng may lower blood sugar levels. One study (https://pubmed.ncbi.nlm.nih.gov/25015735/) found that supplementation with Panax ginseng led to a significant decrease in post-meal glucose levels and an increase in post-meal insulin levels compared to a placebo. If you are taking medications that lower blood sugar, it's important to monitor your blood glucose levels closely when using Panax ginseng.

Here's the thing to keep an eye on with this combo. Your Metaglip is a blood sugar lowering medicine: the glipizide part nudges your pancreas to release more insulin, and the metformin helps your body use it better. Puravive contains Panax ginseng, which in some studies modestly lowered after-meal glucose on its own. Stacked together, the worry is simply that your blood sugar could dip a little lower than expected.

This is rated a minor interaction and it's not a sure thing, so don't panic. Still, watch for signs of low sugar like shakiness, sweating, dizziness, or feeling foggy, especially in the first couple of weeks.
