Linagliptin falls under Antidiabetes Drugs — the group Puravive interacts with.

Puravive contains Panax ginseng. Clinical studies indicate that Panax ginseng may lower blood sugar levels. One study (https://pubmed.ncbi.nlm.nih.gov/25015735/) found that supplementation with Panax ginseng led to a significant decrease in post-meal glucose levels and an increase in post-meal insulin levels compared to a placebo. If you are taking medications that lower blood sugar, it's important to monitor your blood glucose levels closely when using Panax ginseng.

Here's the thing to keep an eye on if you pair Puravive with your Tradjenta (linagliptin). Puravive contains Panax ginseng, and some studies suggest ginseng can gently lower blood sugar on its own, especially after meals. Linagliptin already works to bring your blood sugar down, so stacking the two could add up and push your numbers a little lower than expected.

This isn't a dangerous, dramatic interaction, and the evidence is modest. Linagliptin by itself rarely causes low blood sugar. Still, it's smart to check your glucose more often if you start Puravive and watch for shakiness, sweating, or dizziness. Please loop in your pharmacist or doctor before adding it, and don't change your linagliptin dose on your own.

P-Glycoprotein Substrates

Linagliptin also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for Tradjenta (linagliptin): one of the ingredients in Puravive, quercetin, may block a gut transporter called P-glycoprotein that helps move certain drugs in and out of your system. Linagliptin is partly carried by this pump, so in theory a little more could get absorbed and stay in your bloodstream. Linagliptin is not a prodrug, so more drug would mean a slightly stronger blood-sugar-lowering effect.

Honestly, this is a minor, mostly theoretical concern. The evidence in people is mixed and weak, and linagliptin rarely causes low blood sugar on its own. If you also take insulin or a sulfonylurea, just keep an eye out for shakiness or dizziness. Before starting Puravive, check with your pharmacist or doctor.
