Metformin, Linagliptin falls under Antidiabetes Drugs — the group Puravive interacts with.

Puravive contains Panax ginseng. Clinical studies indicate that Panax ginseng may lower blood sugar levels. One study (https://pubmed.ncbi.nlm.nih.gov/25015735/) found that supplementation with Panax ginseng led to a significant decrease in post-meal glucose levels and an increase in post-meal insulin levels compared to a placebo. If you are taking medications that lower blood sugar, it's important to monitor your blood glucose levels closely when using Panax ginseng.

Here's the thing to know about pairing Jentadueto (metformin plus linagliptin) with Puravive. This supplement contains Panax ginseng, which some studies suggest can gently nudge blood sugar lower on its own. Since Jentadueto already works to bring your glucose down, stacking the two together could add up and push your blood sugar lower than you intend.

This is rated a minor interaction, and the effect is usually mild, but it's worth watching. If your levels dip too low, you might feel shaky, sweaty, dizzy, or unusually hungry or tired. I'd keep an eye on your home glucose readings if you start Puravive, and check in with your pharmacist or doctor before mixing them, especially if your sugars already run low.

P-Glycoprotein Substrates

Metformin, Linagliptin also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the short version: Puravive contains quercetin, and in the lab quercetin can gently slow down a gut transporter called P-glycoprotein. Linagliptin, one of the two medicines in Jentadueto, is moved around by that transporter. So in theory, taking Puravive alongside it could let a little more linagliptin into your bloodstream than usual.

I want to be honest with you, though: this is mostly theoretical and based on small studies that didn't all agree. Linagliptin is also generally a mild, low-risk diabetes drug, so a small bump isn't likely to cause dramatic problems. Watch for things like nausea, stomach upset, or unusual blood sugar readings, and check with your pharmacist or doctor before adding Puravive rather than changing anything on your own.
