Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the gist for Votrient (pazopanib). A few ingredients in Puravive, like Panax ginseng, propolis, and berberine (from Phellodendron), may have a mild blood-thinning, anti-platelet quality. Pazopanib itself isn't a blood thinner, but it can already raise the risk of bleeding and high blood pressure on its own. Stacking a supplement that nudges your clotting in the same direction could add up, meaning easier bruising, nosebleeds, bleeding gums, or more serious bleeding.

I'll be honest, though: this is mostly based on lab and animal studies, not strong human data, so the real-world risk is uncertain. Still, with a cancer drug like this, I wouldn't combine them on your own. Please run it by your oncologist or pharmacist first.

Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Clinical studies indicate that short-term use of Panax ginseng can increase the QT interval (source: https://pubmed.ncbi.nlm.nih.gov/27240158/). However, no changes in the QT interval have been observed with prolonged use (source: https://pubmed.ncbi.nlm.nih.gov/11978148/).

Here's the concern with pairing Puravive and Votrient (pazopanib). Puravive contains Panax ginseng, and pazopanib is already known to affect the heart's electrical timing, something we call the QT interval. When the QT interval stretches out too far, it can rarely set off a dangerous irregular heartbeat. Stacking ginseng on top, in theory, could add a little extra to that effect.

I want to be honest that the evidence here is mixed. Some short-term studies showed ginseng nudging the QT longer, while longer-term use showed no change. So this is a possible, not a certain, problem. Still, because pazopanib alone already carries this risk, please check with your oncologist or pharmacist before adding Puravive. Watch for dizziness, fainting, or a racing heartbeat.

Cytochrome P450 3A4 (Cyp3A4) Substrates

Pazopanib also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and pazopanib (Votrient). Puravive contains Panax ginseng, and your body relies on an enzyme called CYP3A4 to clear pazopanib from your system. Ginseng has shown mixed effects on this enzyme in studies: in some cases it seems to slow it down, and in others it speeds it up. That's why this is hard to predict.

If ginseng slowed CYP3A4, pazopanib could build up, which might mean more of its known side effects, like high blood pressure, diarrhea, fatigue, or changes in liver tests. If it sped things up, pazopanib might work less well against the cancer. The evidence here is weak and indirect, so I wouldn't panic, but given how serious this cancer drug is, please run Puravive past your oncologist or pharmacist first.

P-Glycoprotein Substrates

Pazopanib also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the short version. Puravive contains quercetin, and there's some early lab and small-study evidence that quercetin can block a gut transporter called P-glycoprotein. Pazopanib (Votrient) is carried by that same pump. If quercetin slows the pump, a little more pazopanib could get absorbed, which in theory might raise its levels and push up its known side effects, things like high blood pressure, diarrhea, fatigue, liver enzyme changes, or hair color changes.

I want to be honest: this is theoretical and based on weak, mixed evidence, not real-world reports with pazopanib. But since pazopanib is a serious cancer drug, please don't start Puravive without first checking with your oncologist or pharmacist.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
