Rosiglitazone Maleate falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the situation with your diabetes medicine, Avandia (rosiglitazone). Your liver uses an enzyme called CYP2C9 to clear this drug from your body. Puravive contains quercetin, which can mildly slow that same enzyme down. If the enzyme works slower, rosiglitazone can hang around a bit longer and build up, which may make its blood-sugar-lowering effect a little stronger than expected.

In real life, that could show up as low blood sugar (shakiness, sweating, dizziness, confusion), and possibly more fluid retention or swelling, which rosiglitazone is already known for. The evidence here is fairly weak and based mostly on a small study with a different drug, so this may not be a big deal for everyone. Still, please check in with your pharmacist or doctor before combining them, and keep an eye on your sugar readings.

One study (https://pubmed.ncbi.nlm.nih.gov/25015735/) found that supplementation with Panax ginseng led to a significant decrease in post-meal glucose levels and an increase in post-meal insulin levels compared to a placebo. If you are taking medications that lower blood sugar, it's important to monitor your blood glucose levels closely when using Panax ginseng.

Here's the simple version: Puravive contains Panax ginseng, and some studies suggest ginseng can nudge blood sugar a little lower on its own. Since rosiglitazone (Avandia) is already working to bring your blood sugar down by making your body more sensitive to insulin, stacking the two could add up and push your levels lower than you expect.

If that happens, you might notice signs of low blood sugar like shakiness, sweating, dizziness, or feeling unusually hungry or tired. The good news is this is rated a minor interaction, and the effect is usually mild. Please don't stop or change your rosiglitazone on your own. If you want to try Puravive, just check your blood sugars more closely at first and let your pharmacist or doctor know.

What Is Rosiglitazone Used For?

  • Rosiglitazone is used along with a diet and exercise program and sometimes with one or more other medications to treat type 2 diabetes (condition in which the body does not use insulin normally and therefore cannot control the amount of sugar in the blood). Rosiglitazone is in a class of medications called thiazolidinediones. It works by increasing the body's sensitivity to insulin, a natural substance that helps control blood sugar levels. Rosiglitazone is not used to treat type 1 diabetes (condition in which the body does not produce insulin and therefore cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
