Darunavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist for Prezista (darunavir): your body relies on a liver enzyme called CYP3A4 to help process this medication. In laboratory dish studies, uva ursi seems to slow that same enzyme down. If that happened in real life, darunavir levels could creep up, which might mean more of its typical side effects like nausea, diarrhea, headache, rash, or stomach upset. Keep in mind darunavir is already taken with a booster (ritonavir or cobicistat) that purposely raises its levels, so the math gets tricky.

The reassuring part: this is only test-tube evidence and hasn't been seen in actual people. Still, since HIV therapy is too important to gamble with, please run uva ursi past your pharmacist or HIV provider before starting it.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Darunavir

What Is Darunavir Used For?

Brand name: Prezista

Darunavir treats HIV-1 infection in adults and in children who are at least 3 years old and weigh at least 10 kg (about 22 lbs). It is not a cure for HIV, but it is used as part of a combination antiretroviral regimen to control the virus.

Darunavir must always be taken together with ritonavir — that pairing is required for darunavir to work properly. Both Prezista (brand name) and generic darunavir tablets are approved for this use. Prezista is also available as an oral suspension, which is particularly useful for younger or smaller patients.
