Darunavir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the situation with your darunavir/ritonavir combo. Both of these are cleared by a liver enzyme called CYP3A4. In lab dish studies, uva ursi seems to slow that enzyme down. In theory, that could let these HIV medicines build up a bit more in your body, which might mean stronger side effects like nausea, diarrhea, headache, or a rash.

I want to be honest though: this is only seen in test-tube work, not in real people, so it's a theoretical concern. Ritonavir itself is already a strong CYP3A4 blocker, so uva ursi's small effect may not add much. Since these are important, tightly managed medicines, please check with your pharmacist or HIV provider before adding uva ursi rather than starting it on your own.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the practical takeaway for your darunavir and ritonavir combo. In lab dishes, uva ursi seems to slow down a cellular "pump" called P-glycoprotein, which helps move certain drugs around and out of the body. Since these HIV medicines can ride on that same pump, the theory is that uva ursi could nudge their levels up a bit, which might mean more of the usual side effects like nausea, diarrhea, headache, or stomach upset.

But I want to be honest about how thin this evidence is. This has only been seen in test tubes, never confirmed in real people, and it's rated a minor concern. What I'd worry about more is that uva ursi is only meant for short-term use and can stress the liver, which already matters with these drugs. Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine.
