Indacaterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Indacaterol is a long-acting inhaler you breathe in once a day to keep your airways open if you have COPD. The liver enzyme CYP3A4 is one of the pathways that helps clear it from your body. In test-tube studies, uva ursi seemed to slow this enzyme down, so in theory it could let a little more indacaterol linger. With this drug that might show up as a faster heartbeat, jitteriness, a shaky feeling, or a headache.

The important thing to know is that this was only seen in lab studies, never in real people, so it's a weak, theoretical concern. Indacaterol is also inhaled, so very little reaches the liver to begin with. Still, if you plan to use uva ursi, just run it by your pharmacist or doctor first.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for your inhaler. Indacaterol (Arcapta, Onbrez) is moved around the body in part by a transporter called P-glycoprotein. In lab dish studies, uva ursi seems to slow that transporter down. If that same effect happened in people, a bit more indacaterol could stay in your system, which in theory might nudge up the usual side effects of this long-acting breathing medicine, things like a fast or pounding heartbeat, a slight tremor or shakiness, or a headache.

The honest takeaway: this is only a theoretical, lab-based concern that has not been seen in real patients, and it's rated minor. I wouldn't lose sleep over it, but since uva ursi is meant only for short-term use, it's worth a quick word with your pharmacist or doctor before combining them.
