Uva Ursi and Indinavir

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Uva Ursi interacts with whole categories of medication. Indinavir falls into 2 of those categories, so it can interact with Uva Ursi in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Indinavir

Indinavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for Crixivan. Your body relies on a liver enzyme called CYP3A4 to clear indinavir out of your system. In lab studies (not in actual people), uva ursi looked like it could slow that enzyme down. If that held true in the body, indinavir could build up a bit higher than usual, which might mean more of its typical side effects, things like kidney stones, belly upset, nausea, or a backache from those stones.

I want to be honest about how soft this evidence is. It comes only from test-tube work, so we really don't know if it matters in real life. Still, since indinavir is an important HIV medicine, it's worth a quick chat with your pharmacist or doctor before adding uva ursi, especially in concentrated supplement form.

Minor

P-GLYCOPROTEIN SUBSTRATES

2 of 2

Indinavir also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for indinavir (Crixivan). Indinavir is moved around your body in part by a transporter called P-glycoprotein, which acts like a gatekeeper pumping the drug back out of cells. In lab dish studies, uva ursi seems to block that pump. If that happened in real life, slightly more indinavir could stay in your system, which in theory might mean more of its usual side effects like nausea, belly pain, kidney stones, or higher bilirubin.

The honest part: this is only a theoretical concern seen in test tubes, never confirmed in people, and it's rated minor. I wouldn't lose sleep over it, but since HIV therapy needs steady, predictable levels, run any uva ursi use past your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

The most serious there right now: Activated Charcoal (major), Grapefruit (major), St. John's Wort (major), Acacia Rigidula (moderate) and Adrafinil (moderate).

By severity: 3 major · 109 moderate · 38 minor.

From our pharmacist’s overview on the Indinavir page

“St. John's wort can drop indinavir (Crixivan) levels far enough to cause HIV treatment failure and viral resistance, which is why this list calls for real care before adding any supplement.”

About Uva Ursi

What is Uva Ursi?

Uva Ursi
  • Urinary tract infections
  • Urinary discomfort
  • Mild diuretic
  • Kidney stones
  • Skin conditions

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Indinavir

What Is Indinavir Used For?

Brand name: Crixivan

  • Indinavir is used along with other medications to treat human immunodeficiency virus (HIV) infection. Indinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although indinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
