Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the situation with these two together. Your body partly relies on a liver enzyme called CYP3A4 to clear modafinil out of your system. In a test tube, uva ursi appears to slow that same enzyme down. If that happened in real life, modafinil could hang around a bit longer than usual and feel stronger, meaning more of its typical side effects like headache, jitteriness, a racing heart, trouble sleeping, or nausea.

The important caveat: this is only lab-dish evidence, and it has never actually been shown to happen in people, so I wouldn't lose sleep over it. Uva ursi is also meant only for short-term use. Still, if you plan to take both, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

Modafinil also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist with uva ursi and modafinil. Uva ursi has shown, in lab dish studies only, that it might block a cellular pump called P-glycoprotein, which helps move certain drugs around your body. If that blocking happened in people, modafinil levels could theoretically tick up a bit, which might mean more of its typical effects like jitteriness, headache, trouble sleeping, a racing heart, or nausea.

That said, this is very much a theoretical concern. It has never actually been seen in humans, only in test tubes, so I'd file it under "good to know" rather than "worry." Since uva ursi itself should only be used short-term, just give your pharmacist or doctor a heads-up before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Modafinil Used For?

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
