Saquinavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea. Your body relies on a liver enzyme called CYP3A4 to break down saquinavir (Invirase). In lab dish studies, uva ursi looked like it could slow that enzyme down. If that were to happen in real life, saquinavir could clear out of you more slowly and build up to higher levels than expected, which can mean more side effects like nausea, diarrhea, belly upset, headache, or changes in your heart rhythm.

The reassuring part: this effect has only been seen in test tubes, never confirmed in people, so it stays theoretical for now. Still, your HIV regimen is too important to gamble with, and uva ursi can be hard on the liver. Please check with your pharmacist or doctor before combining them.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the situation with saquinavir (Invirase) and uva ursi. Saquinavir is moved around your body partly by a transporter called P-glycoprotein, which acts like a gatekeeper for how much of the drug gets absorbed and stays in your blood. In lab dish studies, uva ursi appears to block that gatekeeper. If that happened in people, saquinavir levels could nudge upward, which in theory could mean more of its usual side effects like nausea, diarrhea, belly upset, or headache.

I want to be honest though: this is only seen in test tubes, not in actual patients, so it's a theoretical concern. Uva ursi also isn't meant for long-term use. Since your HIV treatment is too important to gamble with, just run it by your pharmacist or doctor before adding uva ursi.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
