Why this applies to Tucatinib

Tucatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea with Tucatinib (Tukysa). Your body relies on a liver enzyme called CYP3A4 to break this medicine down and clear it out. In lab tests (test tube only, not in actual people), uva ursi seems to slow that enzyme. If it did the same thing in your body, your tucatinib levels could creep up higher than intended.

More tucatinib in your system can mean more of its usual side effects, like diarrhea, nausea, mouth sores, hand-foot skin reactions, or changes in your liver lab numbers. I'll be honest with you though: this is only a theoretical concern from lab work, not something proven in humans. Even so, since you're treating cancer, please don't add uva ursi without first running it by your oncologist or pharmacist.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist: Tucatinib (Tukysa) relies partly on a cellular pump called P-glycoprotein to move it around and out of the body. In lab dishes, uva ursi seems to block that pump. If the same thing happened in people, more tucatinib could stay in your system, which in theory might mean more of its usual side effects like diarrhea, nausea, fatigue, hand-foot soreness, or changes in your liver blood tests.

The important part: this is only a test-tube finding, and it has never actually been shown in humans, so I wouldn't lose sleep over it. Still, since you're being treated for cancer and uva ursi itself can stress the liver, please run it by your oncologist or pharmacist before combining them. Don't change your tucatinib on your own.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. About Tucatinib

What Is Tucatinib Used For?

Brand name: Tukysa

Tukysa (tucatinib) tablets are used in combination with trastuzumab and capecitabine to treat adults with advanced, unresectable, or metastatic HER2-positive breast cancer — including those whose cancer has spread to the brain — who have already received at least one HER2-targeted treatment.

Tukysa tablets are also used in combination with trastuzumab alone to treat adults with RAS wild-type, HER2-positive unresectable or metastatic colorectal cancer that has continued to grow after prior chemotherapy. This colorectal cancer use has received accelerated approval, meaning final approval depends on further study results confirming the benefit.
