Heart Guard contains three active ingredients. Organic arjuna (Terminalia) has been used traditionally for heart health.
Organic sahijan (moringa) is a plant leaf commonly used in traditional medicine. Organic Reetha is also included.
The capsule itself is made from vegetable cellulose.
Does it work?
Not established
The evidence supporting these ingredients is limited. For arjuna, the data we hold shows insufficient reliable evidence to rate its effectiveness for obesity, high cholesterol, high blood pressure, and chest pain (angina).
The same is true for sahijan (moringa) — we have insufficient reliable evidence to rate it for iron deficiency anemia, asthma, athletic performance, diabetes, HIV/AIDS, or high cholesterol. In short, we don't yet have solid evidence that either ingredient works for the conditions this product targets.
How safe is it?
Well-documented data
Arjuna is generally well tolerated in short-term studies, though long-term safety data are sparse and quality varies between products. The safety data advise against use during pregnancy and while breastfeeding.
Moringa leaf appears generally well tolerated as food, but robust safety studies are limited. Medicinal doses should be avoided during pregnancy because the root and bark may stimulate the uterus.
For breastfeeding, moringa has been traditionally used to boost milk supply, but safety hasn't been well studied — use caution. Orally, moringa may cause transient diarrhea (about one in four people in a trial experienced it), and rare cases of allergic reactions including anaphylaxis and a serious skin condition called Stevens-Johnson syndrome have been reported.

The most serious finding is moderate-severity interactions affecting how your body processes several classes of drugs — specifically, arjuna and moringa both may slow the breakdown of medications metabolized by your liver's CYP3A4, CYP2D6, and CYP2C9 enzymes, potentially raising drug levels in your blood.

Arjuna carries moderate-severity interactions with blood thinners (anticoagulants and antiplatelet drugs) — it may increase bleeding risk — and with diabetes medications, where it could theoretically lower blood sugar further. It also interacts specifically with chlorzoxazone (a muscle relaxant) and omeprazole (a heartburn medication), both moderate severity.
Moringa adds moderate-severity concerns with thyroid hormone replacement (levothyroxine), which it may reduce, and with P-glycoprotein substrates, a transport system that moves drugs in and out of cells. Moringa's interaction with diabetes drugs is rated minor, as is its unlikely interaction with nevirapine (an HIV medication) and its possible effect on CYP1A2 substrates.

organic Arjuna
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.
Antidiabetes Drugs
Theoretically, concomitant use of Terminalia bellirica or Terminalia chebula with antidiabetes drugs could affect blood sugar control and increase the risk of hypoglycemia.
Animal and in vitro research shows that Terminalia bellirica and Terminalia chebula fruit and seed extract have hypoglycemic effects.
Chlorzoxazone (Parafon Forte, Paraflex)
Theoretically, use of Terminalia chebula may increase the risk of adverse effects from chlorzoxazone.
Animal research shows that enteral administration of Terminalia chebula for 15 days prior to administration of chlorzoxazone increases blood levels of chlorzoxazone and decreases chlorzoxazone clearance. It is speculated that Terminalia chebula reduces the metabolism of chlorzoxazone by inhibiting cytochrome P450 2E1.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.
Omeprazole (Prilosec)
Theoretically, use of Terminalia chebula may increase the risk of adverse effects from omeprazole.
Animal research shows that enteral administration of Terminalia chebula for 15 days prior to administration of omeprazole increases blood levels of omeprazole and decreases omeprazole clearance. It is speculated that Terminalia chebula reduces the metabolism of omeprazole by inhibiting cytochrome P450 2C19.
organic Sahijan
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Levothyroxine (Synthroid, Others)
Theoretically, moringa leaf can antagonize the effects of levothyroxine.
Animal research suggests that moringa aqueous leaf extract might reduce serum triiodothyronine (T3) concentrations by inhibiting the peripheral conversion of thyroxine (T4) to T3.
P-Glycoprotein Substrates
Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Antidiabetes Drugs
Theoretically, moringa might have additive effects when used with antidiabetes drugs; however, research is conflicting.
Animal research shows that moringa can lower blood glucose levels. However, research in humans has not shown consistent blood glucose lowering effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, moringa might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that moringa extract induces CYP1A2 enzymes.
Nevirapine (Viramune)
Moringa leaf is unlikely to have a clinically significant interaction with nevirapine.
Nevirapine is partially metabolized by cytochrome P450 3A4 (CYP3A4). In vitro evidence suggests that moringa inhibits CYP3A4. However, a pharmacokinetic study in patients with HIV shows no change in nevirapine pharmacokinetics when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
