The evidence for these mushroom extracts is limited. For maitake, the data we hold doesn't establish its effectiveness for hay fever, chemotherapy-related anemia, laryngeal cancer, or chronic fatigue syndrome.
Reishi shows possibly ineffective evidence for high cholesterol (hyperlipidemia), and otherwise insufficient evidence for altitude sickness, Alzheimer's disease, or athletic performance. Shiitake appears possibly ineffective for dental plaque and has insufficient evidence for chemotherapy-related anemia, heart disease, breast cancer, or chemotherapy-related nausea.

How safe is it?
Well-documented data
These mushroom extracts are generally well tolerated when taken by mouth. The most common side effects are digestive — diarrhea, nausea, stomach upset, and abdominal discomfort appear in small numbers of trial participants.
Reishi may cause dizziness, dry mouth, itching, or rash. Shiitake can cause bloating or vomiting.
A rare allergic rash cleared on its own in one trial participant taking maitake. Respiratory allergy to mushroom spores, sore throat, and runny nose are possible with reishi.
Raw shiitake mushroom can cause a distinctive skin eruption called shiitake dermatitis, and workers exposed to shiitake spores have rarely developed difficulty breathing and chest pain. Long-term safety hasn't been fully studied.
There isn't enough data on concentrated supplements in pregnancy or while breastfeeding — talk with your doctor or pharmacist for personalized advice on either.

Double-check blood thinners like warfarin (Coumadin) and other anticoagulant or antiplatelet drugs — maitake and reishi both raise bleeding risk. Blood pressure medications (antihypertensive drugs) need a look too — both maitake and reishi can lower pressure further.
If you take diabetes medication, maitake particularly can increase the risk of low blood sugar (hypoglycemia). Immunosuppressant drugs used after transplant should be reviewed because shiitake may weaken their effect.
Medications broken down by the CYP2D6 enzyme pathway may be affected by shiitake, though this hasn't been confirmed in humans.

The most serious concern is with blood thinners (anticoagulants) like warfarin — maitake mushroom may increase their effect, raising your bleeding risk. A case report described one patient's INR (a measure of blood thinning) jump to 5.1 after taking maitake for just one week.

Both maitake and reishi can lower blood pressure, so combined use with blood pressure medications (antihypertensive drugs) could theoretically push your pressure too low. Maitake also interacts with diabetes drugs — it can lower blood sugar, which means your risk of low blood sugar (hypoglycemia) goes up if you're already on diabetes medication.
Reishi has similar theoretical concerns with diabetes drugs, though the clinical evidence is mixed. Reishi at higher doses may also increase bleeding risk with blood thinners or antiplatelet drugs.
Shiitake can interfere with immunosuppressant medications used after transplant or for certain autoimmune conditions. It may also affect how your body processes drugs broken down by the CYP2D6 enzyme in your liver — this hasn't been seen in humans yet, but it's theoretically possible.

Reishi Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Shiitake Extract
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
Maitake Extract
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
