Why this applies to Abrocitinib

Abrocitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for Cibinqo (abrocitinib): your body partly relies on a liver enzyme called CYP2C9 to break this medication down. In lab studies, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme. If that happened in a real person, abrocitinib could clear more slowly and build up to higher levels in the blood, which could mean stronger effects and a greater chance of side effects like nausea, headache, acne, or, more importantly, a more suppressed immune system and higher infection risk.

I want to be honest, though: this is only a test-tube finding, not something seen in people, so the real-world risk is uncertain. Since abrocitinib already affects your immune system, I'd just run the supplement by your pharmacist or doctor before combining them.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's something worth knowing if you're on Cibinqo (abrocitinib) for your eczema. This JAK inhibitor can already nudge your platelet counts down a bit, especially in the first few weeks, which can make bleeding and bruising more likely. Ceylon Leadwort has been shown in animal studies to slow how platelets stick together and to lengthen bleeding time, so stacking the two could in theory add up to a higher bleeding risk.

I'll be honest, this concern comes from lab and animal research, not human studies, so it's far from proven. Still, since abrocitinib already affects platelets, I'd play it safe. Watch for easy bruising, nosebleeds, or bleeding gums, and check with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist with Cibinqo (abrocitinib). It's a JAK inhibitor that purposely turns down part of your immune system to calm severe eczema. The concern with Ceylon Leadwort is that its main compound, plumbagin, has also shown immune-affecting activity in lab and animal studies. In theory, pairing the two could push your immune system in unpredictable directions, either layering onto abrocitinib's effects or muddying how well it controls your skin.

I want to be honest, though: this is theoretical and based only on test-tube and animal research, not real-world human data. Still, since abrocitinib already raises infection risk, I'd rather you not add an unstudied immune-active plant on top. Please check with your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
