Amitriptyline, Chlordiazepoxide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Limbitrol DS combines amitriptyline (a tricyclic antidepressant) with chlordiazepoxide (a benzodiazepine). The amitriptyline part is cleared in good measure by a liver enzyme called CYP2D6. In test-tube research, plumbagin from Ceylon Leadwort appears to slow that same enzyme down. In plain terms, if this enzyme works less efficiently, amitriptyline could build up a bit more in your body than expected.

If that happened, you might notice stronger amitriptyline side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat. I want to be honest though: this is only a lab finding and hasn't been seen in real people, so it's theoretical. Still, since amitriptyline can affect heart rhythm, it's worth running this combination by your pharmacist or doctor first.

Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist if you're taking Limbitrol (which pairs amitriptyline with chlordiazepoxide) alongside Ceylon Leadwort. The amitriptyline part is partly broken down by a liver enzyme called CYP1A2. A compound in Ceylon Leadwort, plumbagin, has been shown in test-tube studies to slow that enzyme down. In theory, that could let more amitriptyline build up in your body, nudging up its usual side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, and a faster heartbeat.

I want to be honest about the strength of this: it's only lab data, and it hasn't actually been seen in real people. So I wouldn't lose sleep over it, but since both of these medicines already make folks sleepy, please run it by your pharmacist or doctor before combining them.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Limbitrol DS combines two active medicines: amitriptyline (an older antidepressant) and chlordiazepoxide (a benzodiazepine). Both are partly broken down by a liver enzyme called CYP3A4. A compound in Ceylon Leadwort, plumbagin, has slowed this enzyme down in lab dishes. The thinking is that if the enzyme works slower, these drugs could clear out of your body more slowly and build up a bit.

If that happened, you might notice stronger versions of their usual effects: more drowsiness, dizziness, dry mouth, constipation, blurred vision, or feeling foggy and overly sedated. Keep in mind this is only a test-tube finding and has never actually been reported in people, so the real-world risk is uncertain. Still, since both ingredients already cause sedation, it's worth running this combination past your pharmacist or doctor before mixing them.
